

About the Role
The Care Navigator supports children, youth, and families by helping them access behavioral health, mental health, developmental, medical, school-based, and community support services. This role is responsible for engaging families, identifying barriers to care, coordinating referrals, assisting with appointment follow-up, connecting families to community resources, and collaborating with clinicians, schools, providers, and internal team members to ensure families receive timely and appropriate support.
Requirements
Bachelor’s degree in social work, psychology, human services, counseling, public health, education, or a related field, preferred. The candidate should have experience working with children, youth, families, or individuals with behavioral health, mental health, developmental, autism, or social service needs. This role also requires a strong understanding of care coordination, family support, community resources, and social determinants of health to help families access appropriate services and supports.
